The Independent Living Centre WA (ILC) provides information and advice, assessment, funding and hire services that enable Western Australians of all ages and abilities to live more independent and fulfilling lives.

We are seeking a person to join our dedicated and experienced RAS team to assess the needs of frail aged persons and people with disabilities, their carers and families. You will be responsible for effective assessments underpinned by a wellness philosophy to ensure individuals who require HACC support maintain their ability to remain within the community. Key duties include assessments, care coordination and support planning, report writing and liaison with referral sources and community organisations.

Suitable applicants will have a Cert IV or tertiary qualification relevant to Human Services and Community Care. Our office is located in Westminster and you will be provided with training.

Salary range commences at $64,190 with generous salary packaging.
