The Independent Living Centre WA (ILC) provides information and advice, assessment, funding and hire services that enable Western Australians of all ages and abilities to live more independent and fulfilling lives.

We are seeking a person to join our dedicated and experienced Regional Assessment Service (RAS) team to assess the needs of frail aged persons and people with disabilities, their carers and families. You will be responsible for active assessments underpinned by wellness and reablement to ensure individuals who may require Community Home Support Program (CHSP) or WA Home and Community Care (HACC) support maintain their independence in their own home.
Key duties include assessments, working with clients to develop goal oriented support plans, report writing and liaising with referral sources and community organisations
Suitable applicants will have a tertiary qualification or a minimum Cert IV relevant to Human Services and Community Care. Our office is located in Westminster and you will be provided with training.
