The Independent Living Centre WA (ILC) provides information and advice, assessment, funding and hire services that enable Western Australians of all ages and abilities to live more independent and fulfilling lives
Managed by ILC, the Commonwealth Respite & Carelink Centre Metro North (CRCC) provides assistance and support to carers who are seeking access to information, respite care and a range of other support appropriate to their individual needs and circumstances.
As Carer Liaison Officer you will work as a member of a small multi-disciplinary team to provide information; liaise with carers to identify and plan their respite needs; coordinate short-term or emergency respite care using a brokerage model of service; and facilitate referral where required to ongoing support that sustains the carer in their caring role.
Applicants with experience working with carers of people with mental illness, disability and/or aged care needs are most suited to this position.
