New HACC Funded Allied Health Service

The Independent Living Centre are excited to announce the introduction of their new community Allied Health Service funded by the WA Home and Community Care Program (HACC). Our Allied Health Team will provide short-term Occupational Therapy and Physiotherapy services in your home with a focus on regaining or maintaining physical, functional and cognitive abilities, which support safety and maximise independence and quality of life.

A Physiotherapist or Occupational therapist may be able to support you/and your carer if you have goals to:

  • Increase independence and safety with daily activities such as showering, dressing, and preparing meals
  • Reduce the risk of falls, improve strength and balance
  • Access the community and participate in social activities
  • Learn strategies to manage memory loss or chronic disease (i.e. associated pain and fatigue)

We offer the Lifestyle-Integrated Functional Exercise (LIFE) Program, a short-term program that reduces the risk of falling by improving strength and balance through everyday activities and routines.

The service is offered to eligible people seeking to receive basic support services under the WA Home and Community Care Program (WA HACC).  This includes frail aged, younger people with a disability and their carers living in the community with an ongoing functional disability.

Please note exclusions to our service include post-acute rehabilitation, soft tissue treatment and individuals who can access more appropriate Allied Health programs (i.e. persons aged under 18 years of age, individuals with a chronic and severe mental health diagnosis).

If you currently receive HACC funded services and feel allied health services may be of benefit for you contact your RAS assessor, or alternatively you can contact Central RAS on 1300 785 415 to be screened for eligibility for HACC services.
